Following a diagnosis of dementia or cognitive impairment it is common to have questions or want to explore aspects of your ongoing care.
Health education and advice about what your particular dementia diagnosis means, how it may change over time and how you can gain support are very important to your ongoing care.
We can provide appointments to help you talk through your diagnosis and what it means for you and we are also able to assist in planning your care going forward.

The approach to ongoing support is individualised as every person has a unique set of medical, personal and family circumstances that are all relevant in optimising your care. Adjusting to a diagnosis of Dementia and cognitive impairment may result in changes to one's sense of self-identity or produce some feelings of insecurity or confidence.
Through a Holistic assessment to understand 'who you are' it allows us to work in collaboration with you to work on what is most important to you. This may be achieved through looking at your strengths and supporting you to maintain independence as long as possible.
